Rhabdomyolysis
Causes:
- Lime or muscle compartment ischaemic for a prolonged period.
- trauma and crush injury
- after overexertion of muscles.
- intense physical exercise
- medical condition: malignant hyperpyrexia or neuroleptic malignant syndrome.
Investigation:
- Creatine kinase (CK) level of > 1000 U/L is highly suggestive,
Management:
- Identification and correction of the underlying cause
- support for multi-organ dysfunction
- forced alkaline diuresis (using intravenous bicarbonate infusion and furosemide) can be used to maintain a good flow of less acidic fluid within the renal tubules and reduce myoglobin precipitation
